Arteriovenous malformations involving the vein of Galen.

چکیده
The vein of Galen is the largest of the deep cerebral veins formed posterior to the pineal body by confluence of the two internal cerebral veins. It curves sharply upward around the splenium of the corpus collosum and drains the deep medially located veins of the diencephalon, basal ganglia, and mid brain. Dilatation of the vein of Galen results from the force of arterial blood either directly from an artery or by way of a tributary vein or veins which receive blood from an artery.2 The dilated vein of Galen is therefore a varix associated with an arteriovenous malformation. In most cases a venous (dural) anomaly can be demonstrated downstream from the draining vein,3 and this, together with the high blood flow into the vein of Galen causes its dilatation. Hoffman et a!4 have described four prominent angiographic patterns of Galenic arteriovenous malformations and related these to the clinical presentation treatment and outcome.

The vast majority of cerebral vascular malformations reported in the antenatal literature are aneurysmal malformations of the vein of Galen. Other types of intracranial vascular malformations are exceptional in fetuses. The authors present a case of prenatal diagnosis of intracranial pial arteriovenous fistula, not involving the vein of Galen and not showing abnormal intracranial anatomic featu...
متن کاملAngiographic findings in two cases of aneurysmal malformation of vein of Galen prior to spontaneous thrombosis: therapeutic implications.
Two cases of aneurysmal malformations of the vein of Galen with later spontaneous thrombosis are reported. Angiograms before thrombosis in both cases showed mural type aneurysmal malformations of the vein of Galen with slow arteriovenous shunts and associated stagnation of contrast in the venous sac secondary to severe outflow restriction. Based on these angiographic findings, the patients were...
متن کاملAntenatal diagnosis of aneurysmal malformation of the vein of Galen.
Vein of Galen malformation (VGAM) results from an aneurysmal aberration with an arteriovenous shunting of blood. Vein of Galen aneurysmal malformations are the most frequent arteriovenous malformations in infants and fetuses. The congenital malformation develops during weeks 6-11 of fetal development. Infants often die from high-output congestive heart failure.

Arteriovenous fistulae between the embryonic vessels situated in the midline are often associated with aneurismal dilatation of the great cerebral vein. These fistulae occur in the telencephalic, diencephalic, and mesencephalic segments of the primitive brain. The evolution of these malformations is described.

Background and Importance: Intracranial pial arteriovenous fistula is a recently described matter as a rare vascular malformation occurring in children. It is usually located in the subpial space in the intracranial cavity with a high-flow shunt. Case Presentation: Authors report on a 2-year-old child with intracranial pial arteriovenous fistula and progressive enlargement of he...
